|District Reintegration Plan Survey and Public Forum|
I would like to invite any interested community members to join us via Zoom on July 27, 2021 at 1pm to discuss our Fall 2021 Reintegration Plan that continues our focus on a safe return to in-person instruction and continuity of services. As you are well aware, our District 125 Reintegration Plan has been in the works since July, 2020 but continues to be affected by a number of factors such as changes in the CDC and IDPH guidance as well as a myriad of state and local orders that continue to change at a dizzying pace. It is our intent to keep the lines of communication flowing so that you are aware of our Reintegration Plans throughout the District. Please fill out this short survey and join our public forum to discuss the survey results. We look forward to hearing your thoughts, concerns, and ideas for improvement!

Please take a moment to fill out the electronic versions of our Emergency Contact Information as well as the Self-Certification form which is required before students are allowed to return to school. This will also save you time from having to fill out a hard copy of it at Registration. Per our Smart Restart Plan, Parents/Guardians and the schools will be doing daily temperature checks on every student and staff member. We will send out periodic reminders since this form only has to be submitted once. It is an expectation that temperature checks are completed every morning before your child goes to school. If your child is found to have a temperature of 100.4 or greater, you (or your designated emergency contact) will need to pick them up within one hour of notification from the school.
