Health Forms and Requirements

Current district immunization data is required to be submitted to the State Board of Education by November 15th of every year.

** IHSA PreParticipation Form is required to participate in sports in Hamlin.

Updated Immunization Information

Pre-Kindergarten students are required to have one (1) pneumococcal vaccine.

Kindergarten and 6th-grade students are required to have two (2) Varicella injections (chicken pox).

6th-grade students are required to have Tdap and Meningococcal immunizations. The Illinois Department of Public Health recommends that all persons 10 years of age and older get vaccinated with Tdap to protect against the ongoing threat of whooping cough.

6th-grade students are required to have the Hepatitis B vaccination series (3 shots) completed.

New for 2015: Students in 6th grade will be required to have the Menactra or MC4 vaccine (meningitis). A 2nd dose will be required for the 12th grade. We recommend you discuss this with your healthcare provider.
